- A legible photocopy of a valid driver's license, passport, or other government issued ID
- Photocopy of the front and reverse sides of all card(s) used on your account
- A legible photocopy of a recent statement from the credit card your initial purchase was made with as well as both side of the credit card
- A copy of any utility bill showing the same address as the player's sign up form
